    Seeking Compassionate Home Health Registered Nurse for home health contract inBaton Rouge.

    Responsibilities:

    • Conduct comprehensive patient assessments, develop individualized care plans, and provide skilled nursing interventions to promote optimal health outcomes.
    • Collaborate with interdisciplinary teams, including physicians, therapists, and caregivers, to coordinate holistic care and ensure continuity of services.
    • Educate patients and their families on disease management, medication administration, and self-care techniques to empower them in their health journey.
    • Utilize telehealth platforms and digital health tools to monitor patient progress, facilitate virtual consultations, and enhance communication with the healthcare team.

    Requirements:

    • Valid Registered Nurse (RN) license
    • Minimum of 1 yearof home health experience with OASIS
    • Proficiency in conducting comprehensive assessments, developing care plans, and administering skilled nursing interventions.
    • Strong communication, interpersonal, and organizational skills.
    • Comfortable utilizing technology for documentation, communication, and remote patient monitoring.
