    Description

    JOB PURPOSE:

    Serves a part of the agency's medical team through a collaborative relationship with a mutually agreed upon assigned physician and is responsible for providing psychiatric assessment, medication management and documentation as required and approved by regulatory and paneled payer sources. Clinician will provide patient, family, staff and community education within his/her scope of care. Clinician will work closely with the medical and executive leadership to provide recommendation for program development and continuous quality improvements.

    1.0 ESSENTIAL JOB FUNCTIONS:

    1.1 Conducts initial psychiatric evaluations, treatment planning and periodic re-evaluations on adult patients. Prescribes mediation base upon evaluation and scheduled medication checks as medically indicated but not longer than a 3-month interval. Conduct evaluations on an emergency basis as needed.

    1.2 Chairs bi-weekly treatment team. Provides medication education. Participates in family education services when necessary. Responsible for delivering culturally competent psychiatric care. Psychiatric services may be provided to adolescents to the extent of the physician's comfort level and experience level by mutual agreement with Chief Executive Officer.

    1.3 Must be proficient in the Agency EHR. Assessments and progress notes are to be competed. dated and signed by the end of the workday in which the services was performed. Lab reports must also be dated and signed. Medication orders must be signed on the day of order. At minimum, the documentation must be completed in a manner which meets the standards established by regulators, licensing bodies and managed care/insurance companies. Reviews health screening questionnaires and recommends follow-up referrals/examinations.

    1.4 Clinician will work closely with the medical and executive leadership to provide recommendations for program development, staff development and continuous quality improvement measures. Staff, community patient and family education recommendations will also be provided.

    1.5 Meets agency expectations regarding productivity (i.e. level of direct patient care provided) and adheres to generally accepted practices guidelines for conducting initial evaluation, re-assessment and medication checks (in terms of time allotted.) Participates in initial orientation with Chief Executive Offer and other clinical, administrative and support staff. Participates as needed in internal agency committees.

    2.0 Supervisory Responsibilities: Functions as a member of multidisciplinary treatment team. Must be able to communicate effectively with a variety of personnel and participate in an atmosphere of mutual respect for the skills and experience represented on the team. Coordinates with other providers, such as, Community physicians and agencies.

    5.0 QUALIFICATION: Board certified. Maintains professional credentials and provides documentation to be an approved provider for various payer sources.

    6.0 CERTIFICATES, LICENSES, REGISTRATIONS: Pennsylvania MH CRNP medical license. Act 33 and 34 Clearances required. Must maintain a current driver's license and vehicle insurance.

    7.0 PHYSICAL DEMANDS:

    While performing the duties of this job, the employee is frequently required to sit, talk and hear. The employee is occasionally required to stand and walk. The employee must frequently lift and/or move up to 10 pounds. Specific vision abilities required by this job include the ability to adjust focus.

    8.0 WORK ENVIRONMENT: The noise level in the work environment is usually quiet.
