- JOB SUMMARY
- The Case Manager may be a Registered Nurse or Social Work professional who coordinates all aspects of the care of individual patients related to utilization of services, planning and coordinating discharges, and obtaining resources and coordination of those resources. The Case Manager's role focuses on facilitation and collaboration in the multidisciplinary planning and coordination of the patient's care across the healthcare continuum. The Case Manager organizes the sequence of patient care to attain maximum levels of effectiveness and efficiency to reach desired quality, clinical and cost outcomes. The Case Manager organizes multiple strategies to insure the care of all patients is managed with minimal overlapping roles and processes. The Case Manager participates in a team composed of the physician/provider, nurse(s), social service, and other direct and indirect providers and payers to coordinate patient care in a complex health care system. The Case Manager works to meet complex patient needs to ensure the patient receives medically appropriate services. Case Managers review medical records to ensure patients receive appropriate levels of care. The Case Manager facilitates admission and discharge processes.
- JOB QUALIFICATIONS
- Education:
- Registered Nurse with a license to practice in Wyoming or a Bachelor's Degree in Social Work.
- RN candidates: BSN, Master's degree or Doctoral degree preferred. May be a graduate from an Associate's Degree in nursing program with completion of a BSN from an accredited school required within four (4) years of accepting the case management position.
- Social worker candidates: Bachelor's Degree in Social Work required, Master's degree or Doctoral degree in social work preferred.
- Two years' experience in utilization review or case management preferred.
- Certification in Case Management such as CCM or ACM required within 5 years.
- Job Knowledge and Skills:
- Five years clinical experience with two years' experience in case management/one-year utilization management experience (InterQual Autobook) preferred.
