    DEPARTMENTAL SUMMARY:


    The St Charles Health System Pharmacy provides a broad range of inpatient and outpatient pharmaceutical care services to ensure safe and effective medication use.


    POSITION OVERVIEW:


    The postgraduate year-one (PGY-1) resident will participate in an organized, directed, postgraduate training program for one year in hospital pharmacy practice.

    During this one year training experience, he/she will design, recommend, monitor, and evaluate patient-specific pharmacotherapy and hospital-wide patient care initiatives.

    The pharmacy resident will also assist Pharmacy Leadership in other hospital wide medication management initiatives through education of health professionals and students, leadership activities, and at least one research project.


    ESSENTIAL FUNCTIONS AND DUTIES:
    Under the direct supervision of a precepting pharmacist, resident pharmacists will:


    Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including, but not limited to, advising physicians on issues concerning drug therapy, the inherent toxicity of drugs and their side effects, as well as assisting in the prescription of appropriate doses.


    Coordinate the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with physicians' prescriptions; ensures compliance with federal, state and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records.

    Mentor and trains others in training including practical experiences.


    Maintain liaison relationships with medical and nursing staff; provides timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission Standards.


    Conduct and evaluates medication histories, assesses compliance and suggests modifications so as to achieve desired outcomes; instructs patients in the proper use of prescribed drugs and makes Patient Care Rounds with physicians to evaluate patient progress.


    Individualize medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of Pharmaceutical Care to patients within areas of direct responsibility and assisting others in the department with the same.

    Extract pertinent information from patient/other sources for the patient's medical management plan.

    Operate pharmacy computer system(s) and other equipment.

    EDUCATION:

    Required:
    Bachelor of Science degree in Pharmacy.


    Preferred:
    PharmD.


    LICENSURE/CERTIFICATION/REGISTRATION:

    Required:
    Must be able to obtain current registration by the Oregon Board of Pharmacy within 90 days of hire.

    PERSONAL PROTECTIVE EQUIPMENT:

    PHYSICAL REQUIREMENTS:
    Continually (75% or more): Standing, walking, carrying 1-10 pounds, Operation of Motor Vehicle* (*courier).

    Frequently (50%): Lifting 1-10 pounds, pushing/pulling 1-25 pounds, grasping/squeezing.


    Occasionally (25%): Sitting, bending, climbing stairs, lifting/carrying 11-25 pounds, Use of clear and audible speaking voice and the ability to hear normal speech level.

    Rarely (10%): Stooping/kneeling/crouching, reaching overhead, pushing/pulling 25-50 pounds, keyboard operation.

    Never (0%): Climbing ladder/stepstool, lifting/carrying 25-50 pounds, operation of motor vehicle, whispered speech level

    Exposure to Elemental Factors
