    The RN Patient Care Manager is a Registered Nurse who is responsible for the oversight of home health clinical services including private duty, skilled nursing, and home infusion clients.

    The RN Patient Care Manager performs new patient admission assessments, and mentors the RN infusion team, as well as the nursing and CNA field staff.

    • We offer flexible work schedules on a variety of assignmentsResponsibilities
    • Provides care as defined within the scope of the state's nurse practice act.
    • Makes the initial evaluation visit and regularly reevaluates the client's nursing calls to clients and field supervisory visits.
    • Prepares clinical and progress notes and summaries of care.
    Conducts clinical record review.

    • Supervises, educates and coaches other nursing personnel/care-giving staff.responsible for introductions and caregiver training for new clients in start-up period.
    Maintains RN on call schedule.

    • Plans and implements in-service and training programs.
    Implement improvement processes that impact clinical qualityCoordinating and scheduling CPR program

    • Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel.
    Requirements

    • Current unrestricted RN license in state for which care is being provided

    Preferred:
    Bachelor's degree in Nursing. Must have graduated from a National League of Nursing accredited program

    • Minimum three (3) to five (5) years of clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility, preferred.
    • Reliable car/auto insurance
    • Understands regulatory compliance with state and federal regulations
    • Adhere to HIPAA and maintain client confidentiality
    • Demonstrated ability to supervise and direct staff.
    • Possess exceptional nursing assessment and infusion skills
    • Excellent interpersonal skills.
