    • Responsibilities

    • Review resident information and evaluate its impact on assessment and treatment.
    • Conduct and document a thorough functional assessment of the resident using standardized procedures.
    • Develop and implement an individualized treatment plan based on resident needs.
    • Comply with evaluation, treatment, and documentation guidelines.
    • Instruct assistants in treatment procedures and provide close supervision.
    • Enter progress notes in the resident record after each treatment session.
    • Revise treatment plans as needed throughout the treatment process.
    • Document the course of treatment in a clear discharge summary.
    • Orient residents to program services and inform them of potential risks during procedures.
    • Work with nursing staff, residents, and families to maximize residents' use of functional abilities.
    • Instruct residents and caregivers about exercise programs and therapeutic procedures.
    • Assist in the referral process when a resident requires additional services.
    • Maintain current knowledge of community resources.
    • Attend rehabilitation team conferences and participate in quality improvement processes.
    • Adhere to facility policies and procedures.

    Qualifications

    • Bachelor's or Master's degree in Physical Therapy.
    • Current license in the state of practice.
    • Prefer experience in a setting serving the same age/type of resident population.
    • Certification and demonstrated competency in identified areas of specialization.
    • Excellent communication and interpersonal skills.
    • Demonstrated competency in applying physical therapy principles and methods.
    • Ability to assess resident needs and develop comprehensive plans of care.
    • Knowledge of accreditation standards and compliance requirements.
    • Ability to complete physical tasks safely, including standing, pushing, lifting, stooping, reaching, and balancing.
