    The Lead Physical Nutritional Management Plan Coordinator (PNMPC) works under the general supervision of the Habilitation Therapy Director or other designated staff based upon the needs of the State Supported Living Center (SSLC).

    The Lead PNMP Coordinator provides oversight and guidance to services delivered to individuals by direct care staff in the areas of physical and nutritional management, oral and enteral eating, positioning, mobility, communication, and other related PNMP services.

    Conducts competency-based training of staff and mentors new staff. Monitors therapeutic activities, communicating problems and recommendation for improvement to Habilitation Therapy staff.

    Monitors the availability, condition, and proper use of assistive equipment and ensures appropriate availability and condition of PNMPC instructions and illustrations.

    Performs and leads the work of the others to perform duties including distribution of assignments, providing coaching to improve the quality of work and acts as an interface between direct support professionals and Habilitation Therapists.

    Trains, leads, mentors and guides others in the implementation of planned client therapy programs, and in the set up and preparation of equipment for therapeutic activities and ensures equipment is well functioning.

    Participates, and assists others to participate, in PNMP meetings and clinics, NMT (Nutritional Management Team) meetings, and PST/HST (Personal Support Team/Health Status Team) meetings.

    Maintains client records, ensures proper inventory levels, completes data system entry, ensures the distribution of Physical Nutritional Management Plans (PNMPs), in-services and other documentation under the direction of the Habilitation Therapists.

    Provides new employee orientation and refresher training as indicated and oversees the training of new employees with specific individuals served in lifting, mealtime procedures and other PNMP related activities/plans.

    Work hours are normally day or evening shifts with variable responsibility for night shift activities.
    This position is required to drive a state-owned vehicle.

    Essential Job Functions:

    Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.

    Performs and leads the work of direct support professionals to perform duties including providing daily work assignments, provides on-the-job training and coaching and communicates problems or issues to the Habilitation Therapy supervisor or designee.

    Monitors all aspects of PNMP related programming, quality of implementation, and staff compliance and reports findings to QIDP, Unit RN, and HT to ensure continued effectiveness of programs.

    Provides orientation, training and mentoring to new direct support professionals and refresher training to tenured staff.

    Oversees the training of new employees in topics such as lifting, mealtime procedures, and other PNMP related activities/plans demonstrating with specific individuals in accordance with their plans.

    Provides competency-based training, coaching and mentoring to ensure direct care staff is well prepared and knowledgeable about activities outlined in the PNMPs to ensure Plans are carried out as prescribed.

    Acts as an interface between direct support professionals and Therapists to ensure good communication of individual's plans, needs and issues.

    Gives input into development of PNMPs for assigned individuals to meet needs agreed upon by the IDT.
    Serves as a member of the PNMP Team to ensure PNMP information is accurate. Ensures appropriate attendance and participation by direct support professionals at assigned meetings and committees.

    Maintains, and trains direct support professionals to maintain, pertinent documentation in PNMP notebooks in a timely manner to keep others informed of PNMP changes, and trains staff in changes.

    Assists in implementing planned client care programs. Prepares, and trains others to prepare, departmental equipment and inventory.
    Leads the monitoring of meals and snacks to ensure safe eating practices.
    Provides assistance, as needed, in transporting of individuals going on scheduled activities and in emergency situations.
    Performs, and oversees others to perform, accurate data entry, and timely maintenance of client records

    Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.

    Such participation may require an alternate shift pattern assignment and/or location.

    Knowledge Skills Abilities:


    • Knowledge of rehabilitative programs for individuals with developmental disabilities
    • Knowledge of physical nutritional management plans.
    • Knowledge of oral and enteral eating, positioning, mobility, communication, and other related PNMP services.
    • Skill in the use of computers for data entry.
    • Skill in training and coaching others to perform job duties.
    • Ability to administer first aid or cardiopulmonary resuscitation (CPR) once trained to do so.
    • Ability to express ideas in writing with attention to spelling, grammar and organization.
    • Ability to follow a series of oral directions.
    • Ability to express ideas verbally in a clear and orderly fashion;
    • Ability to establish and maintain rapport with co-workers and individuals served.
    • Ability to collect, organize, and enter information into databases.
    • Ability to provide assistance to clients in Habilitation Therapies.
    • Ability to maintain flexibility in work schedule.
