QUALIFICATIONS :
Licensing and certification:
Must hold a current, unencumbered license in this state as a registered nurse or licensed practical nurse. Must be registered with the State Board of Registration in this state.
Education:
Must be a graduate of an accredited educational institution (school of nursing) or a graduate of a state approved school of practical nursing
Experience:
At least two years nursing experience preferred especially in a long- term care facility
SPECIAL DEMANDS:
Must possess skills to work with resident, resident's family, and the care assessment team. Must be able to communicate and work effectively with other Clinical Care Coordinators.
JOB KNOWLEDGE :
Thorough knowledge of the operations of a long-term care facility; thorough knowledge of Federal and State regulations for assessment of long-term care residents; thorough knowledge of Medicare case-mix requirements; thorough knowledge of minimum data set requirements; good knowledge of administrative procedures; good knowledge of federal, state, local and professional policies, rules, regulations, and procedures as they relate to the nursing service of a skilled nursing facility; operate within scope of Nurse Practice Act; good knowledge of the care of long-term geriatric residents; good knowledge of automated system; familiarity with health practices and procedures; ability to coordinate and direct the activities of a multi-disciplinary staff; ability to maintain cooperative working relationships; ability to communicate effectively both orally and in writing; ability to identify problems and needs in order to formulate and implement appropriate care planning techniques; possess sound and mature judgment; objectivity; resourcefulness; tact; and thoroughness
DUTIES AND RESPONSIBILITIES :
1.
Coordinate and schedule MDS data collection based on federally mandated time frames, or in response to resident events, to assess residents' functional status and develop comprehensive care plans.
2.Coordinate multi-disciplinary teams comprised of: Social Service, Dietary, Nursing, and Therapy departments, to promote and facilitate continuity and coordination of care between disciplines, ensure compliance with MDS standards and policies, and develop and implement comprehensive care plans.
3. Complete all MDS assessments for nursing. 4. Participate in planning and adjustment of Medicare assessment scheduling. 5. Validate the entire MDS once printed and ensures documentation is present to support the MDS. 6. Write Rap Summaries. 7.evelop and conduct educational programs to orient staff to the MDS and multi-disciplinary care planning process and provide on-going training as required.
8. Complete Medicare screen upon admission. 9. Complete and maintain Resident Roster and Census. 10. Update Care Plans daily. 11. Complete a Discharge Planning Sheet on all residents discharged from the facility. 12. Attend meetings of supervisory and administrative staff. 13. Assist with studies related to improvements of nursing care. 14.Provide nursing care on the unit when staffing necessitates, and cooperate with other members of the medical care team in coordinating residents' total needs.
15. Demonstrate knowledge of each resident's historical and diagnostic data in relation to its impact on nursing care. 16. Prepare for and attend team and family conferences in a professional, supportive manner as assigned. 17. Communicate with nursing and clinical personnel on the implementation of the resident care plan. 18.Adhere to Resident's Bill of Rights and Confidentiality of Resident Information, as well as to the Resident Abuse Reporting Law.
19. Complete risk assessment sheets for each resident. 20. Implement the falling star program for residents, who have a high risk of falling. 21. Maintain the MDS Completion, New Order Review, and Restorative Nursing Care logs. 22. Complete Flow Sheet Monitors. 23. Make daily rounds of resident halls to identify any potential problems. 24. Assess restraint needs of residents, and if needed obtain a restraint consent form from resident's family. 25. Handle any emergency situations that may arise in the facility.OTHER WORK VARIABLES :
Working hours:
Day shift: From 8:00 a.m. to 5:00 p.m. M-F with flexibility to work weekends or do floor work or as approved by supervisor.
