Job Summary
We are seeking a compassionate and proactive Resident Care Coordinator to join our dedicated team in providing exceptional care to residents in assisted living, memory care, and senior care settings. In this vital role, you will serve as a central point of contact for residents, their families, and our care team, ensuring personalized support that promotes independence, dignity, and well-being. Your leadership will help coordinate manage care plans, and foster a warm, safe environment where residents thrive. This paid position offers an exciting opportunity to make a meaningful difference in the lives of individuals with diverse needs, including those with Alzheimer's disease, dementia, developmental disabilities, autism, and other health challenges.
Responsibilities
- Develop and implement individualized care plans tailored to each resident's unique needs, preferences, and medical conditions.
- Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, mobility assistance.
- Monitor vital signs and medication administration in accordance with healthcare protocols and HIPAA regulations.
- Coordinate with nursing staff and healthcare providers to ensure comprehensive patient care and accurate documentation using EMR systems.
- Manage behavioral issues through effective behavior management strategies and conflict resolution techniques.
- Conduct regular assessments of residents' physical and emotional well-being; update care plans accordingly.
- Facilitate communication between residents' families and the care team to ensure transparency and involvement in care decisions.
- Oversee safety procedures including first aid response, infection control practices such as cleaning protocols, and emergency preparedness.
- Responsible for overseeing staff schedules, oversee call offs/obtaining staff coverage
- Covering weekend on call every other weekend.
Experience
- Prior experience working in assisted living facilities, nursing homes, or home care environments is highly preferred.
- Demonstrated knowledge of dementia care, Alzheimer's support strategies, memory care techniques, and working with individuals with disabilities or developmental challenges.
- Experience with medical terminology, medication administration, vital signs monitoring, and case management is essential.
- Familiarity with EMR systems for documentation; proficiency in HIPAA compliance is required.
- Background in Nursing, Med tech or caregiving roles that involve direct support for seniors or individuals with special needs is advantageous.
- Responsible for overseeing staff schedules, oversee call offs/obtaining staff coverage
- Covering weekend on call every other weekend.
Job Type: Full-time
Pay: $ $33.00 per hour
Expected hours: 40 per week
