- Conduct annual home health risk assessments for a Medicare Advantage health plan.
- Evaluate prior and current medical history, medical conditions and medication history.
- Provide education on medical conditions and medications.
- Identify any gaps in care and provide recommendations for resolution.
- No creation of treatment plans or prescribing of medications is required.
- Nurse Practitioner License in AR.
- Current CPR certification (BLS).
- Board Certification through AANP or ANCC.
- 6 months of current NP experience is preferred.
- MSN degree from an accredited school of nursing.
- Flexible shifts between Monday and Sunday
- Create your own schedule.
- Minimum weekly or bi-weekly hourly commitment.

Job DescriptionMinimum 1+ years nurse practitioner experience required.Here at Sunbelt, we are currently seeking a licensed Nurse Practitioner (NP) to conduct in-home assessments in Winamac, ARand throughout the PulaskiCounty on a per diem basis. The qualified candidate will be responsible for covering MC Advantage Member's annual In-Home Health assessment throughout the county.
Each visit will take about 45- 60minutes. No Prescribing responsibilities, visits are not meant to replace PCP or existing doctors appointments.
This position offers a great opportunity fora highly motivated and detail-orientated individual looking to work independentlywith flexible hours with the ability to self-schedule.
Availability can be divided amongst any days between Sunday - Saturday, 8am - 8pm making it possible to fit around most FT schedules.
Candidate should have 6 months experience employed as an NP and/or Home Health experience/exposure in healthcare career.
