- Oversees the administrative and clinical supervision of behavioral health staff dedicated to integration within Primary Care Teams.
- Assists the Director of Behavioral Health with behavioral health integration program strategy, design and development.
- Under the direction of the Director of Behavioral Health, and in collaboration with medical department leadership, has primary responsibility for developing behavioral health initiatives designed to improve the health outcomes of identified patient populations (depression, diabetes, obesity, chronic pain).
- In collaboration with the nursing and primary care team leadership, assists in the design of the daily operations of the teams ensuring coordination and collaboration of behavioral health integration specialists with case management and primary care team members.
- Provides behavioral health counseling, advocacy, and referrals to community resource services to primary care patients at 35% effort.
- Performs other related duties as required.

- Graduate-level degree in mental health or a related social services field.
- Independent licensure as an LICSW or LMHC.
- Minimum of 7 years of relevant clinical experience with at least 4 years of post-licensure experience in providing direct client care across clinical settings.
- Minimum of 2 years of clinical and administrative supervisory experience within a healthcare setting.
- Minimum of 2 years of experience with integrated behavioral health programming.
