- Collaborate with physicians, case managers, and other medical personnel
- Work in Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree or commensurate experience, NP licensure
- Current RN and NP licensure with prescription authority
- Board certified by AANP, ANCC, or AACCN
- Current BLS, ACLS or CPR certification
- Minimum 1 year as a Nurse Practitioner
- Strong computer skills, excellent communication, flexibility
- Ability to travel, valid driver license, automobile insurance for residence state
- Travel to meet patients at their location

PAY: $90.00 or $95.00 per completed Assessment
Location: Home, SNF, Telehealth in Toledo, Ohio
