    Patient Retention Specialist


    A professional who assists in the prevention of acute care hospitalization for patients who are identified as high risk for re-hospitalization.

    Patient Retention Specialist will also assist the Director of Quality Assurance in obtaining records needed to complete ICD-10 coding.


    Qualifications:

    • Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing;
    • A qualified health care professional.
    • Three to five (3-5) years of health care experience, preferred.
    • One (1) year experience in home care, preferred.
    • Ability to exercise initiative and independent judgment.
    • Ability to work with individuals, to enlist cooperation of many people to perform/achieve a common goal.

    Responsibilities:

    • Understands and adheres to established Agency policies and procedures.
    • Participates in studies and research and other administrative functions as assigned.
    • Communicates with high-risk patients at identified timeframes.
    • Notify Clinical Manager of any signs/symptoms or report of exacerbations for high-risk patients that may warrant a visit.
    • Observes confidentiality and safeguards all patient related information.
    • Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regard to time involved.
    • Serves as a resource person to employees.
    • Immediately reports any accident, incident, lost articles or unusual occurrence to the Director of Clinical Services/Clinical Manager.
    • Attends pertinent continuing education programs other than routine in services and shares information with staff.
    • Communicates with current/discharged patients to ensure that quality services were provided at identified timeframes.
    • Obtains additional medical records for purposes of ICD-10 coding.
    • Upload additional records to the EMR and notify appropriate staff.

    Working Environment:
    Works indoors in the home health office.

    Lifting Requirements:
    Ability to perform the following tasks if necessary:

    • Ability to participate in physical activity.
    • Ability to work for extended period of time while sitting or standing and being involved in physical activity.
    • Moderate lifting.
    • Ability to do extensive bending, lifting and standing on a regular basis.
