- Provide ongoing support and encouragement to clients, fostering a safe and non-judgmental environment.
- Connect clients with appropriate community resources and services to enhance their treatment outcomes.
- Coordinate and communicate with other professionals, such as therapists, doctors, and social workers, to ensure holistic care for clients.
- Regularly monitor progress and reassess treatment plans to ensure the effectiveness of interventions.
- Advocate for clients and empower them to actively participate in their recovery process.
- Conduct home visits or accompany clients to appointments when necessary.
- Maintain accurate and up-to-date documentation of all client interactions and interventions.
- Participate in case conferences, team meetings, and professional development activities.
- A bachelor's or master's degree in social work, psychology, counseling, or a related field.
- Possess strong interpersonal and communication skills to collaborate effectively with clients and team members.
- Experience working in a mental health or social services setting is preferred.
- Knowledge of community resources and the ability to facilitate client access to these resources.
- Ability to maintain confidentiality and demonstrate ethical behavior in all situations.
