Description
Full Time (Monday-Friday 8AM-5PM)Millennium Physician Group is seeking to hire a full-time / Case Manager to join the ACO team.
The Case Manager focuses on client-centered services that link clients and their family members with health care, and other home or community-based services to ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care.
They are responsible for assessment, planning, coordination, advocacy, education, monitoring, and accurate, timely documentation in all program areas.They demonstrate knowledge of growth and development and possess the ability to assess, interact, communicate, and care for age-specific patient populations.
We also offer opportunities for growth, as well as a great team atmosphere that empowers you to seek better ways to deliver service and take ownership of outcomes in providing quality service and support.
3+ years of Clinical experience with 1+ years of case management experience preferred.To perform this job successfully, an individual should have strong computer skills, especially in Excel, and knowledge of EMR systems; 3 weeks PTO & 7 paid holidays
