    COMPANY MISSION

    We are an At-Home provider for the treatment of Substance Use Disorder (SUD = Alcoholism and Drug Addiction) and Co-Occurring Mental Health Condition(s).

    We are strong advocates of what is often referred to as the Patient-Centered Medical Home (PCMH) Model, which has inspired us to create an At-Home approach to treating addiction. Combined, our medical and clinical professionals have over 100 years of experience treating the Disease of Addiction.

    We believe it is extremely important for individuals struggling with addiction to alcohol and/or narcotics, as well as their loved ones, to understand this medical condition is considered to be a chronic ("incurable") disease. We can not cure you, but our At-Home approach is proving to be quite effective with "arresting" the disease long-term.

    We subscribe to the "traditional treatment bubble" idea -- please see out website on our philosophy -- and believe that keeping patients in a real life setting while we are constantly there to help them overcome relapse triggers is far more efficacious long-term than the consistently similar programming and routines offered within an institution.

    Although our full addiction treatment program involves 3 Phases that last 52 weeks, depending on where you are in your addiction recovery, we can offer our services in an a la carte fashion. We can offer Phase I Detox services on its own, but we caution that such services are short-term in nature and the probability of relapse is high if only Detox services are offered. We offer Phase II Intensive tele-health counseling and tele-medicine services which offers a continuum of care, immediately following Phase I Detox services. Lastly, our Phase III Electronic Monitoring Phase allows the patient to remain in constant touch with our clinicians and medical personnel to give the patient the highest probability for long term sobriety.

    Responsibilities:

    This is a fully remote position for tele-medicine and evaluation services for PHMNP residing in TN.

    Responsible for the appropriate psychiatric evaluation and assessment of all clients, in conjunction with working with Psychiatrist to prescribe appropriate pharmacotherapy meds. The PHMNP provides accurate and appropriate medication assisted therapies and subsequent monitoring. Within TN, the PHMNP will directly report to a Board-Certified Psychiatrist and Addictionologist.

    · Responsible for performing psych evaluations on client shortly before/after client intake

    · Ensures all documentation completed within 24 hours of patient visits.

    · Consistent communication with clinical staff to ensure continuity of care.

    · Responsible for the education of clients pertaining to psychopharmacology medication and narcotic use/misuse interactions and contra-indications.

    · Responsible for accurate and timely documentation in the Electronic Medical Records system (Best Notes as a plus)

    · Prescribes and monitors appropriate medications for clients within the in accordance with all policy, procedures, and state and federal regulatory standards

    · Ensure consistent and collaborative communication with other health professionals for effective delivering service to clients.

    · Other duties as assigned by Supervisor.

    Qualifications:

    · Available for work schedule with 24hrs notice.

    · Ideal candidate has a minimum of 2 years experience with Mental Health and/or SUD populations in either outpatient or hospital setting

    · Must be licensed to practice as APN in State of Tennessee

    · Certifications for practice as PMHNP; preferred

    · Ability to self-direct, manage schedule, and adhere to professional standards of conduct.

    · Complete documentation in accordance with applicable healthcare standards in a professional and timely manner.

    This position is a 1099, allows candidate to work for multiple entities, and pays at competitive rates.
