    We believe in driving a high-performance culture in order to product maximum results and more importantly, provide the highest level of patient care for our residents The RN or LPN is responsible to the Unit Manager and its clients for rendering nursing care in assigned area in terms of individualized customer needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Registered Nurse.

    *Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned customer/family as evidenced by documentation and observation of positive customer care outcomes
    Initiates and leads team conferences in development of individualized nursing care plans
    Contribute to the development of the customer/significant other plan of care based on the identified customer problems/needs
    Assist in the evaluation of the effectiveness of the nursing care administered relative to the customer/ significant other identified problem/needs
    Document pertinent observation and intervention as appropriate in the EMR
    Perform selected tasks related to customer care and the customer's environment in a safe manner
    Therapy procedures on those customers receiving peripherally inserted Intravenous fluids, pursuant to LPN scope of practice.

    Maintain and coordinate communication among customer/significant other, team members, and physician as evidenced within the EMR, client, and health care team feedback.

    .*Graduate of an accredited LPN/RN program
    *A minimum of one (1) year of nursing experience in a SNF
