    Park Place of Elmhurst is an elegant five-star community conveniently located across the street from Edward Elmhurst hospital. Here at Park Place of Elmhurst, we offer a continuum of care with the security of lifecare. Our residents also have access to assisted living, short-term rehabilitation, skilled nursing, and memory support right on campus.

    We are seeking a hardworking full-time LPN to support carrying out our mission to deliver quality care to our residentsHours available: 7am-3:30pm*Must be eligible to work two holidays a year and two weekends a month.

    Summary of essential job functionsResponsible to provide direct nursing care to the clients and manage the day-to-day activities performed by the Certified Nursing Assistants working as a team.

    Ensures that client needs are met within professional standards of practice according to physician orders.
    Administers medications and treatments according to physician orders.

    Assists clients in meeting their goals through assessment and monitoring of clients care needs and interaction with the interdisciplinary team including the physician.

    Provides education and guidance to clients and their families on disease process and management.

    Manage the CNAs in accordance with current rules, regulations, guidelines, policies and procedures established by PLS.Document in nurses' notes in an informative and descriptive manner that reflects the care provided for the resident, as well as the resident's response to care.

    Identify the resident before administering any medication or treatment.
    Ensure adequate medical supplies are available to meet the medical needs of the patients. Report the needs to the Director of Nursing and/or Administrator.
    Provide direction and leadership to the CNA staff.

    Receive and review nursing report at the beginning of the shift and compile nursing report at the end of the shift.

    Asses the residents change in medical condition to the proper authority at that time.
    Ensure the care given is in accordance with federal and state regulations, as well as the resident's wishes.
    Monitor vital signs in accordance with the resident's treatment plan. Requirements Must be licensed as LPN in the State of ILPrevious experience in long-term care is a plus.
    Minimum one year of medical/surgical experience.

    The LPN must be a person who is licensed as a Licensed Practical/Registered Nurse in the State of IL and be in good standing.

    The nursing license must be active and unencumbered in accordance with the department and professional regulations.
