    The Care Coordinator works closely with the behavioral health patient, doctor and other clinic staff to ensure the patient has the resources and tools necessary to improve their mental health by providing access to high quality physical and behavioral healthcare in order to help reach wellness and recovery of the whole person.

    The Care Coordinator works closely with other team members to perform screenings, assessments and intake documentation in order to successfully enroll patients into services.

    This position is also responsible for closing the gap in treatment by linking patients to access multiple types of services within the community and provide follow up to ensure the patient receives services with those providers to achieve their recovery goals.

    This position works independently, under limited supervision, reporting major activities through periodic meetings.

    A Bachelor's degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistance, gerontology, special education, educational psychology, early childhood education or early childhood intervention or a bachelor's degree with at least 30 hours of coursework in the previous fields.


    Experience Required:
    At least 1 year experience in mental health field preferred.

    Qualifications Required:
    Coordinate patient care with other internal or external providers.

    Utilize community resources to assist patient in recovery from mental or physical illness and to provide access to services such as financial assistance, medication assistance food/housing, job placement or education.

    Complete, track and follow up on PAP applications.
    Provide PAP and Sample transaction slips to financial clerks.
    Maintain accurate PAP information in IPM Pharmacy Benefit Manager database.
    Meet unit performance measures or targets.
    Comply with the Abuse, Neglect, and Exploitation policy and reporting requirements.
    Adhere to the Code of Conduct and Standards of Behavior policy requirements.
    Answer phone, collect phone messages and respond to requests timely and accurately.
    Display professionalism when representing PermiaCare and the program in the community.
    Maintain compliance with legal requirements and company policies and procedures.
    Complete all training as assigned prior to due date.
    Fill in for other MH staff as needed.
    Complete medical records requests.
    Obtain prior authorizations from third party payers when needed.
    Participate in community activities and/or attends community meetings as needed.
    Ability to collect and organize data to identify patient needs.
    Ability to express self clearly and effectively, orally and in writing.
    Excellent computer skills, including Word, Excel, Outlook, and Electronic Health Records (EHR).
    Ability to maintain highly confidential information.
    Ability to plan and schedule work and implement directives without constant supervision.
    Model professionalism by appropriate dress, language, ethics and work habits.

    This position may require travel to agency program sites, community and residential sites, and/or locations outside the PermiaCare catchment area.

    Operating office equipment
