The Matrix Comprehensive Health Assessment (CHA) involves reviewing a member's current health, medical history, medication adherence, social environment and other risks during an in-home visit. This assessment provides unmatched insight into a member's overall well-being that can be difficult to capture during routine office visits.
The role requires conducting adult / geriatric assessments including medical history, diagnosis and treatment,health education,, physician referrals,case management referrals,, follow-up & clear documentation according -Matrix guidelines-protocols. Administering & ordering point-of-care lab-based testing as needed also work with physicians case managers social workers family members key caregivers ancillary medical personnel collaboratively working Primary Care Physician (
