    Job Description

    Develops and implements wellness programs for Our Lady of the Lake employees and the community, collects and evaluates data from various screenings and evaluates and interprets various methods of health screenings.

    Responsibilities

    1. Clinical Practice and Care Management
      1. Develops, coordinates, and delivers innovative, results-oriented organizational wellness programs and activities which support the long-range goals of the hospital and meet specific needs of the community.
      2. Serves as a resource to employees, business and industry, and the community by determining health needs and offering high quality health management services that reduce employee health risks and increase job performance while promoting employee physiological, emotional, and social well-being.
      3. Analyzes information gathered from assessments and offers comprehensive wellness offerings as needed.
      4. When in contact with pediatric, adolescent, or geriatric patients, performs all patient care duties in a manner that demonstrates understanding, respect, and compassion for the physical and psychological needs relevant to the age of the patient served.
      5. Responsible for referring at-risk individuals to case management services or providing the necessary follow-up in a timely manner. Provides referral resources for at-risk individuals to appropriate outside agencies or facilities.
    2. Communication and Collaboration
      1. Develops positive relationships between the hospital and community agencies, special interest and civic groups, and professional affiliations by acting as a hospital representative. Works through civic groups and public education activities to improve community health and foster the hospital's image as a leader in the health care industry.
      2. Coordinates health and wellness events, as well as performs all medical tasks necessary, i.e. finger sticks, phlebotomy, injections, etc. Collects and compiles data for outcome measurement.
    3. Quality
      1. Promotes the quality and efficiency of his/her own performance through participation in staff educational programs, approved continuing education courses, and specialized skill training programs.
    4. Professional Development
      1. Participates in wellness educational programs, in-service meetings, and training sessions in an effort to share one's own expertise with others and to further the quality of education and personal growth provided to new personnel, volunteers, and interning students.
      2. Performs other duties as assigned or requested.
    Qualifications

    Experience, Education, Training, Special Skills, and Licensure :
    • 2 years in Health Wellness/Prevention
    • Graduate of an accredited nursing program
    • Current Louisiana State RN and BLS
