    Overview of the Role

    Under the direction of the Director of Nursing, the Licensed Practical Nurse (LPN) plays a vital role in delivering quality care to residents of assigned neighborhoods. This professional caregiver is accountable for planning, implementing, and evaluating all aspects of resident care, including physical, psychosocial, and environmental needs.

    Key Responsibilities

    • Accurately documents care according to nursing practice standards.
    • Administers medications promptly and accurately, providing treatments to residents as per physician orders.
    • Maintains a safe environment and provides physical comfort to residents.
    • Carrries out physician/provider orders in a timely manner.
    • Initiates, implements, and evaluates care plans/MDS on an ongoing basis.
    • Completes admission and discharge procedures accurately.
    • Coordinates staff and resident care to meet individual needs.
    • Fulfills all policies, annual mandatory in-services, required meetings, and other conditions of employment.
    • Performs additional duties as assigned.

    Requirements and Qualifications

    • Current CT LPN license is required.
    • At least 2 years of full-time clinical experience in nursing is necessary.
    • Ability to communicate effectively in English is essential.
    • Preferred qualifications include knowledge of geriatric nursing care, principles of nursing, and physical evaluation skills.
    • Basic knowledge of human relations skills is required to interact effectively with residents, their families, and CHHC staff.

    Work Schedule

    Available Per Diem and Flex Schedules:

    1st Shift: 7:00 am - 3:30 pm

    2nd Shift: 3:00 pm - 11:30 pm

    3rd Shift: 11:00 pm - 7:30 am

    Compensation

    Competitive Pay Rates:

    Per Diem: $30.00/hour + shift differentials ($2.00/hour weekday evenings, nights & weekend days, $3.00/hour weekend evenings and weekend nights)

    Flex Per Diem: $35.00/hour (requires 3 years of skilled nursing experience) + shift differentials ($2.00/hour weekday evenings, nights & weekend days, $3.00/hour weekend evenings and weekend nights)
