- Licensed Practical Nurse (LPN/LVN) responsible for skilled nursing care under RN supervision.
- Delivery and coordination of quality patient care in compliance with physician orders.
- Observes and assesses patient condition, reporting changes to the supervisor or physician.
- Timely documentation of all activities, assessments, and coordination of care.
- Participate, implement, and update nursing care plans.
- Takes appropriate nursing action based on assessments.
- Recognizes and intervenes in changes in patient needs.
- Ensures infection control measures are followed.
- Educates patients and families on disease process, selfcare, and prevention strategies.
- Maintains knowledge through educational programs and skill evaluations.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance.
- Six (6) months of recent experience in a clinical care setting.
- Home health experience.
- Must be able to travel, lift 50 pounds, and reposition patients and equipment.
- Various physical activities, including walking, standing, bending, and climbing stairs.
