    LIF, Inc. (Love is Foundation) is a non-profit organization dedicated to enriching the lives of misguided male adolescents. LIF Inc's sole purpose is persuading troubled young men their life has meaning and purpose. We provide a nurturing environment via residential homes and committed professionals. Our values and methods celebrate success as well as teach accountability. We strive to establish a sense of normalcy; our young men attend school, participate in sports and programs, and more importantly, receive daily mentorship from our House Directors and Life Coaches. We've put young men on the path to become highly trained professionals with unwavering morals.

    Our House Directors are responsible for the management and oversight of a residential home which includes but is not limited to communication with agencies for placement of eligible young men, supervisory duties for Life Coaches, and managing the well-being of the young men residing in the home. We are looking for experienced Case Coordinators/Case Managers with supervisory experience.

    POSITION: LIFE COACH – Full-Time - Mon-Fri, 8am-4pm

    JOB DESCRIPTION:

    • Empower young men (ages and help to establish realistic plans to achieve individual goals; adjust short-term goals and strategies every 30 days
    • Maintain maximum capacity of residential home
    • Coordinate and monitor health visits including health assessment, dental, physical, medical evaluations and therapy appointments
    • Complete necessary documentation to facilitate referrals to collaborative outside specialty providers to ensure continuity of care (such as child mental/behavioral therapy)
    • Utilize electronic data system to document and track all case management services; (such as Youth Program Plans, Progress and Case Notes, Critical Incident and Monthly Reports)
    • Teach youth appropriate life skills and help work through negative behaviors
    • Monitor progress notes (ie, in document system) daily to ensure notes are entered every day by every shift. When notes are missing will be responsible for ensuring Life Coaches get progress notes entered within 24 hours
    • Monitor progress notes and critical incident reports of Life Coaches to ensure that notes are valid, consistent and compliant
    • Dispense resident's medications and properly document medications given
    • Build positive relationships with youth DAILY; (goal of every day)
    • Transport youth to and from appointments and social events; safely and timely
    • Understand the importance of safety and how to prevent critical incidents
    • Supervises Life Coaches; responsible for their performance and professional development; carries out administrative functions associated with management of Life Coaches
    • Reports to LIF Inc Director; carries out vision of Director
    • Completes and submits reports

    QUALIFICATIONS:

    • Must be able to work with 'male' adolescents with behavioral problems
    • Supervisory experience, preferably in the field of social work
    • Ability to make routine decisions and determine proper action based on experience within the limits of organization's policies and procedures
    • Ability to work effectively with people of diverse cultures, ages and economic backgrounds
    • Strong administrative/clerical skills including basic computer skills
    • Flexibility to work overtime and some holidays
    • Must be at least 21 years of age
    • Must have a current driver's license (good driving record)
    • Must pass a KBI and FBI background check
    • Must have effective organizational skills and ability to handle multiple activities with changing priorities simultaneously
    • Degree in Psychology, Sociology, Social Work, Human Development & Family, Criminal Justice and or Education preferred
