- Provide expert clinical care to inpatients with serious illness, including pain and symptom management, emotional support, and communication of treatment goals and options.
- Collaborate with the interdisciplinary team, including physicians, nurses, social workers, chaplains, and other healthcare providers, to develop and implement patient-centered care plans that align with patients' goals and preferences.
- Conduct thorough assessments of patients' physical, emotional, social, and spiritual needs and develop appropriate care plans accordingly.
- Communicate effectively with patients, families, and the healthcare team to facilitate shared decision-making and ensure that patients' values and preferences are respected.
- Provide education and support to patients and families regarding disease prognosis, treatment options, and end-of-life care planning.
- Coordinate and facilitate family meetings and care conferences to address patients' goals of care, advance care planning, and end-of-life decisions.
- Participate in quality improvement initiatives, including performance improvement projects and palliative care program development.
- Provide emotional support and counseling to patients and families, addressing their psychosocial, emotional, and spiritual needs.
- Collaborate with other specialties, including primary care, oncology, geriatrics, and other relevant disciplines, to provide comprehensive palliative care to patients with diverse diagnoses.
- Maintain accurate and up-to-date documentation in the electronic medical record, ensuring compliance with regulatory requirements and best practices.
- Medical degree from an accredited medical school
- Board certification and fellowship in Palliative Medicine
- Active medical license in the state of Texas
- Excellent clinical skills and ability to manage complex medical and psychosocial issues
