Job DescriptionThe Opportunity:
As a registered nurse care coordinator, you will be responsible for the coordination of care for patients with various complex conditions, treatments, and care plans.
You will manage patient care through the continuum of care and ensure quality of care and services and will collaborate with a diverse multidisciplinary team to provide oversight of care, treatment plans and interventions.
As an informal leader, be confident in your skills, ask questions, and hold the care team accountable for quality patient outcomes.
To be successful in this position, you will need to have great communication skills, ability to develop relationships easily, strong teamwork and a passion for collaborating with diverse teams with different experience and backgrounds to ensure quality patient centered care is delivered.
You will perform as the patient's advocate and will need to possess confidence in your skills to make sure decisions are made that are best for the patient.
Shift:
Full time. This position is Monday through Friday with a weekend rotation.
