- This position does NOT include any travel.
- Contractors complete "medical case file reviews" from the office.
- What they'll do is review a 17 page assessment for a client already on LTC Medicaid who is residing in the community.
- The contractor will then use the information contained in the assessment to determine whether the client still meets the medical criteria for our program
- Each case will require a detailed write up of the client's history, current circumstances as well as justification as to how/why they still meet medical criteria for the program.
- Training will be through job-shadowing and connection with seasoned full time employees.
- We also have video and online trainings.
- Draws conclusions from reading/reviewing medical records, and then writes corresponding case- notes which highlight the key-points.
- Utilizes established guidelines and internal protocol to determine if client meets medical eligibility criteria based on review of medical records and client psychosocial history.
- Applies knowledge of aging process, activities for daily living and medical conditions to written case-notes.
- Justifies medical eligibility recommendation through written narratives which summarize client's current situation, limitations and needs.
- Presents case-notes and medical eligibility recommendations to Registered Nurse for approval, and adjusts to professional feedback provided by Registered Nurse and/or Supervisor
- The candidates must meet the minimum qualification of:
- A bachelor's degree or higher from an accredited college or university with a major in a related field,
OR - professional experience totaling four (4) or more years in a related field
OR - a combination of education and professional experience totaling four (4) or more years in a related field.
