Job Summary: As a PACE Physical Therapist, you will be a key member of our team, delivering top-notch physical therapy services to our elderly participants. Your role involves evaluating, planning, and executing rehabilitative programs to tackle mobility issues, enhance functional abilities, and improve overall well-being. Your expertise plays a vital role in enhancing the health and independence of our participants, enabling them to lead fulfilling lives in their communities.

Responsibilities:
Collaborate with the interdisciplinary team (IDT) and conduct various assessments
Contribute to quality assessment and performance improvement activities
Provide updates on participants' therapy progress to the team
Evaluate the need for adaptive equipment and durable medical equipment
Participate in different meetings as required
Deliver treatments, ensuring safety measures are followed
Create special programs based on participant needs
Educate staff, participants, and families about available services
Train and supervise other staff on therapy programs
Complete necessary documentation
Adjust schedule to meet participants' needs

Education & Training:
Minimum of one year's experience working with elderly population
Bachelor's degree in physical therapy
Current California license and CPR certification required

Skills & Abilities:
Ability to handle mental and emotional stress with personal integrity
Work independently and collaboratively with team members
Excellent communication and organizational skills

Working Conditions: Physical Therapists in a PACE program work in various settings, including the PACE center, participants' homes, and community-based locations. The PACE center offers therapy rooms and specialized facilities, while home visits require therapists to adapt to different environments for personalized care.
