**Summary**
The primary role of a Pinnacle Home Care Clinical Assistant is to ensure the morale, passion for clinical excellence, focus & performance of their clinicians. As a Pinnacle leader, it is expected that this will be accomplished by a laser focus to teach, develop, motivate, and MAKE THEIR TEAMS BETTER every day. This focus will ensure our leaders ability to always answer the question, "Who Have I Made Better Today". Additionally, the Clinical Assistant is responsible for ensuring that patient care is coordinated and managed appropriately.
**Responsibilities**
Schedules patients for entirety of visit plans at the beginning of each episode with consistent clinician ensuring continuity of care
Prepares and manages daily and weekend open schedules to ensure adequate clinician efficiency and patient visit plan completion
Ensures submission of weekly availability by PRN staff and contractors
Collaborates with HR Business Partners identified areas of opportunity
Identifies staffing trends and areas of opportunities. Works collaboratively with Field Business Office Managers, HR Business Partners, and office staff to ensure appropriate staffing levels
Communicates and collaborates with afterhours teams for pending staffing needs
Reviews and interprets after hour call logs at the start of shift and performs follow up, as necessary. Maintains on call recordkeeping locally at branch for survey readiness
Complies with the referral standard operating procedures to process incoming referrals timely and accurately
Continually strives to reach 24-hour admission from time of receipt of referral with each patient
Requests further patient documentation as is necessary in collaboration with Clinical Manager
Identifies and resolves challenges that may contribute to patient's non-admission to agency. Pinnacle believes that each patient that is referred to service should be educated to benefit of home health services, out-of-pocket costs if applicable, and other influences that may drive a patient's refusal of care. Our patients deserve the opportunity to be provided necessary service by allowing the opportunity to make educated decisions
Requests initial authorizations from the Authorization team and follows up on pending requests
Reviews all Admission consents for accuracy and tracks completion
Promotes the use of patient portal, physician portal and Synzi
Promotes and supports LIVE QA process and compliance with clinicians
Reviews outstanding documentation daily that exceed Pinnacle standards. Identifies and assist clinicians in overcoming challenges in relationship to late documentation and collaborates with Clinical Manager to resolve timely
Completes the CA checklist electronically on Teams daily
Prepare and conduct daily stand up and stand down, completes electronic report in Microsoft Teams
Assist with facilitation of branch weekly orientation for new hires to include coordinating precepting schedules
Manages hospitalized clients in accordance with Facility Hold List SOP and collaborates with care coordinators regarding patient status consistently
Manage electronic and manual faxes in accordance with eFax SOP
Manage and assist with customer phone calls and ensure timely response and attention to customer needs
Assists with case conferencing to include preparing patient lists, running appropriate reports, development of the agenda, alerting and reminding staff of meeting, verifying attendance, and recording applicable entries on time sheets
Maintains patient and employee confidentiality in accordance with HIPAA regulations and Agency standards
Assists with identifying staffing trends and barriers for opportunities for improvement to be identified and assists in creative solutions in conjunction with the DCS/CM
Utilizes a consistent focus on the Sacred Seven to provide and great work and operating environment

**Qualifications**
Education/Experience
High School graduate
One year of healthcare experience, home health preferred. Scheduling experience preferred
Knowledge, Skills and Abilities
Ability to work under pressure
Ability to solve problems and create solutions
Demonstrates excellent observation, organization, verbal, and written communication skills
Proficient in Computers, Microsoft, EMR and Technology
Knowledge of governmental regulations and ACHC standards
