    EDUCATION and/or EXPERIENCE:

    • Medical School
    • Residency

    CERTIFICATES, LICENSES, REGISTRATIONS:

    • Current California medical license
    • Current Board Certification
    • Current DEA license
    • Current CPR
    APC supervision when requested
    Speaking and hearing sufficient to communicate effectively by phone or in person, at normal volumes

    • Vision adequate to read correspondence, computer screen, forms, etc.
