- Assess the needs of individuals and develop service plans to achieve successful outcomes.
- Link and coordinate services to ensure continuity of care.
- Provide advocacy and support to individuals and their families.
- Collaborate with community resources and providers to ensure access to necessary services.
- Bachelor's Degree in a human services field.
- One year of experience in the mental health field.
- Certification as a QMHP, QIDP, and/or CMHP, or be licensed in the State of Michigan.
