Job Summary

AuSable Valley Community Mental Health Authority is seeking a skilled Case Manager to join our team. The successful candidate will be responsible for providing advocacy, coordination, and linkage of services to individuals with mental health needs.

Key Responsibilities:

• Assess the needs of individuals and develop service plans to achieve successful outcomes.
• Link and coordinate services to ensure continuity of care.
• Provide advocacy and support to individuals and their families.
• Collaborate with community resources and providers to ensure access to necessary services.

Requirements:

• Bachelor's Degree in a human services field.
• One year of experience in the mental health field.
• Certification as a QMHP, QIDP, and/or CMHP, or be licensed in the State of Michigan.
