    General Information:

    Job title:
    Licensed Practical Nurse (LPN)

    Location:
    Elroy Family Medical Center - Elroy, WI

    Schedule:
    Full-time, 80 hours per pay period; Monday-Friday, 7:00am-4:30pm (depending on day of week)

    Weekend rotation:
    No Weekends


    Holiday rotation:
    Paid Holidays


    Position Summary:
    The Licensed Practical Nurse is responsible for facilitating practitioner efficiency and quality patient care.

    Position Responsibilities:


    Provide direct and individualized nursing care to an assigned group of patients based on the application of scientific principles, under the supervision of a registered professional nurse.

    Assist in relevant health teaching for patient and family.
    Share responsibility for the planning, implementation, and nursing care.
    Participate in team meetings, sharing knowledge, information, and ideas among team members.

    Prepare, administer, and record medications in accordance with the policy regarding the administration of medications by a licensed practical nurse.

    Perform complex nursing activities as competence increases.
    Participate in emergency care.
    Record information concisely, accurately, and completely.
    Attend scheduled Patient Care Staff Meetings.
    Conform to Clinic department dress code.
    Demonstrate flexibility and adaptability to schedule changes; maintain good work attendance; assist other staff in completion of duties.
    Document observations, data collection, treatments, etc. in EHR.
    Seeks opportunity for continued learning.
    Perform other duties as requested.

    Position Requirements:
    LPN license required.
    BLS within 1 month of hire.
    Must maintain a valid, current driver's license.
    1+ years of related work experience preferred.
    Exceptional accuracy and attention to detail required.
    Knowledge, Skills, & Abilities
    Intermediate proficiency with computers is required.
    Knowledgeable regarding medication administration.
    Comprehensive nursing skills.
    Strong quantitative and analytical competency.
    Self-starter with excellent interpersonal communication and problem-solving skills.
