    Position Description

    The Program Specialist 2 - Home & Community Based Services position performs administrative, direct, and in-home supportive services including but not limited to duties which involve performing limited in-home services, medication reminders, Glucose levels, Blood Pressure checks, medical appointment set-up and various HCBS supportive services to enable the elderly and disabled to have a full range of services in their current living arrangement.

    Metropolitan Social Services (MSS) is in the business of helping Nashvillians who are experiencing hardship. Last year the department assisted more than 3600 new clients in addition to its ongoing case management customers. Services include counseling, information and referral, case management and other assistance.     Typical Duties
    • Provides appropriate welfare checks as referrals are made and provides the follow-up to all designated parties.
    • Provides minimal in-home support services to eligible customers. (Set -up medicines, make appointments, check mail, take out trash, etc.)
    • Provides minimal shopping assistance (grocery and pharmacy) to customers w/in 5-mile radius (5-10 items max).
    • Makes appropriate case management referrals to the appropriate MSS unit when and if needed.
    • Provides emotional support to customers and encourage them to be as independent as possible.
    • Maintains and stays abreast of certifications and trainings that directly related to elderly, disabled, mental illness, trauma informed care and Home and Community Based Services.
    • Develops and maintains a routine visitation schedule for customers.
    • Completes, monitors, and submits all medically documented logs (Glucose, Blood pressure).
    • Coordinates with the triage of the HCBS unit staff to ensure that wrap around services are met and the appropriate plan is put in place and monitored by the team.
    • Works in conjunction with the health department, and any other metro departments to ensure the health and safety of the seniors and vulnerable customers.
    • Ensures the safety and the confidentiality of the customer is always kept.
    • Ensures that all actions, and interactions are documented properly and submitted as requested.
    • Attends all meetings, trainings, in-services, and workshops as required.
    • Performs any other duties as assigned.
    Minimum Qualifications

    Bachelor's Degree from an accredited college or university.

    LICENSE REQUIRED
    Valid Driver's License may be required for some positions in this classification.

    Note: Per Metro Ordinance No. SO All employees of the Metropolitan Government shall be residents of the State of Tennessee or become residents of the state within six (6) months of employment as a prerequisite to employment with the Metropolitan Government.
    Preferred Experience, Knowledge, Skills, and Abilities
    • Certified Nurses Training
    • General knowledge of the needs of elderly and disabled persons
    • Knowledge of the rules and regulations governing food service and sanitation
    • Knowledge of basic first aid and CPR
    • Two (2) years of experience working with elderly and/or disabled population
