- Responsible for discharge planning for inmates receiving mental health services. Develop and maintain a procedure for compliance with contract requirements for release prescriptions and recurring prescriptions following release.
- Review all new admissions and meet with patients and complete discharge planning evaluation to determine all discharge planning needs as indicated by treatment team.
- Assist inmates who need follow up in order to continue essential medications, chronic care, and/or their treatment plan by seeking appointments with willing community resources/health providers. Ensures common understanding of admission criteria, eligibility and enrollment requirements when applicable.
- Supports inmate clients by providing required applications and assistance in confirming eligibility, as well as by referring them to assigned agency and correctional facility program.
- Acts as educational liaison and provides consultation to medical personnel regarding discharge planning process, applicable regulations, and accreditation standards.
- Communicate all discharge planning issues with all members of the treatment team.
- Responsible for creating and maintaining relationships with community agencies providing services to persons with mental illness.
- Develop and utilize a discharge resource book identifying community resources i.e. social service resources available in the community for patients.
- Demonstrate accurate and timely documentation according to guidelines.
- Prepares and submits statistical data, monthly activity and annual reports as directed.
- Assist patients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
- Assist in the orientation/training of new Team Members.
- Ensure compliance with regulatory standards.
- Current CPR (Healthcare Provider)
- Ability to pass a background check
- Bachelor's degree in Psychology/Social Work, valid unrestricted LCSW license, OR a Mental Health Counselor license? (Required)
