    Care Coach Connect RN Case Manager Amarillo, TX

    Job Description:


    The Care Coach Connect RN/Case Manager plays a critical role in coordinating patient care and providing ongoing nursing assessment and coordination with interdisciplinary team members.


    The primary responsibilities include:


    • Conducting medical home visits for established Care Coach Connect patients, performing hands-on exams at each visit.
    • Providing urgent/acute visits for established Care Coach Connect patients, aiming to keep them out of the hospital and receive the best medical care at home.
    • Ongoing assessments of the impact of life-limiting and/or chronic illness(es) on patients' physical, functional, psychosocial, and environmental needs.
    • Assessing caregiver burnout and providing community resources as needed.
    • Implementing individualized plans of care and recommending revisions as necessary.
    • Performing procedures such as wound care, phlebotomy, and injections (B12, vaccines).
    • Conducting advanced care discussions with patients and their families, documenting their wishes in the electronic medical record.
    • Initiating preventive and rehabilitative nursing procedures when appropriate.
    • Evaluating patient and family/caregiver responses to care and recommending alterations to the plan when necessary.
    • Solving problems by gathering and reviewing facts, selecting the best solution from identified alternatives.

    Key Responsibilities:


    • Telephonic nursing follow-up and case management when necessary.
    • Serving as a resource or consultant for LVN/MA staff.
    • Attending educational offerings to stay current on chronic medical conditions and complying with licensing requirements.
    • Establishing trusting relationships with patients, caregivers, clinic staff members, and providers.
    • Collaborating with providers to recommend policies, procedures, and standards affecting Care Coach Connect Patient care.
    • Exhibiting professionalism and courtesy with all patients, physicians, and coworkers.

    Requirements:


    We are seeking a Registered Nurse with a current license to practice in the state of employment. Additional requirements include:


    • Current BLS certification or must obtain within 30 days of employment.
    • 3+ years of experience in a physician's office, clinical, hospice, or hospital setting.
    • Demonstrated experience of excellent nursing skills.
    • Proficient computer skills, including Microsoft Word, Excel, Access, and Outlook.
    • Ability to interact productively with individuals and multidisciplinary teams.
    • Reliable transportation for daily travel to various locations as assigned (up to 80% of the time).
    • Valid driver's license within the state of work.

    PREFERRED QUALIFICATIONS:


    Hospice and Palliative Care Nursing certification, 2+ years of hospice experience, experience with home visits, and knowledge of palliative and hospice medicine are preferred qualifications.


    Physical & Mental Requirements:


    Ability to lift up to 100 pounds, sit for extended periods, stand for extended periods, use fine motor skills to operate office equipment, properly drive and operate a company vehicle, and receive and comprehend instructions verbally and/or in writing.
