This organization is a leading provider of integrated medical services caring for individuals from post-hospitalization to home in various settings ranging from skilled nursing facilities (SNFs), assisted living facilities (ALFs), independent living facilities (ILFs), group homes, and private homes. They bring quality medical care to the geriatric population, as well as people with complex conditions, special needs, and with intellectual and/or developmental disabilities. Their providers make a dramatic impact in increasing quality of life and safeguarding the dignity of those they serve. They are currently hiring due to growth in their Columbia market, where they're in need of a provider to serve a skilled nursing facility.
POSITION DESCRIPTION:
The new Nurse Practitioner will see patients in one (1) skilled nursing facility, doing rounds and attending to acute needs on a part-time basis, working three (3) days per week during regular hours.
