Ambulatory Care Pharmacy Specialist roles are open in multiple disease areas: rheumatology, cardiology, infectious diseases, neurology
The Ambulatory Care Specialist will work closely with a diverse team to provide optimal care for patients in our medical centers, emphasizing rational, safe, evidence-based, and cost-effective therapy. Alongside direct patient care, the clinical pharmacy specialist will actively engage in committees and reviews of medications, clinical guidelines, and related policies for managing specific medical conditions treated at the clinics. Additionally, they will conduct clinical research, advance practice projects, and initiatives for improving medication processes and pharmacy practice quality and compliance.
Qualifications
Education:
· B.S. or Pharm.D. Required. PharmD preferred.
Licenses/Certifications:
· Current CT Pharmacist license or eligibility within 45 days
Experience:
· PGY2 Pharmacy Residency in the relevant disease state (Ambulatory Care, Infectious Diseases, Cardiology, etc.) is highly desirable
· Previous work experience demonstrating dedication to patient care and continuous professional development can be a valuable alternative to formal PGY2 residency training.
· Strong customer service and communication abilities
· Previous experience with EPIC systems is advantageous.
