    Referral Coordinator

    About Us:

    Founded in 1970, East Valley Community Health Center is a Federally Qualified Health Center (FQHC) providing personalized medical, dental, vision, and behavioral health care through a community-based network.

    Job Description:

    • Demonstrate ability to prioritize and handle urgent referrals.
    • Enter pertinent medical information into the order management section in NextGen.
    • Work with the healthcare team to ensure prompt processing of referrals.
    • Attend assigned training either offsite or onsite.
    • Demonstrate dignity with kindness and compassion to all patients, visitors, and staff.
    • Scan all referral requests, authorizations, and consult notes into the patient's electronic medical record.

    Requirements:

    • Bilingual English/Spanish preferred.
    • Excellent interpersonal skills to care for patients continually.
    • Excellent communication skills both verbal and written.
    • Proficient in Microsoft Word, Excel, and Data Entry.
    • Able to work with minimal supervision.
