We aspire to aid the growth of our company by welcoming the most qualified and deserving candidates aboard. Optima is seeking a medical assistant for our Care Management department. This individual will be expected to monitor a specific group of patient's care and support them in achieving short/long term health goals. The Care Manager will coordinate with providers to ensure patients' needs are met. A working knowledge of the clinical field is desired for this position. This is a Hybrid position, please note first week of training will take place in Scottsdale, AZ.

    Care Manager Responsibilities:

    • Manage a roster of patients who are enrolled in the Hybrid Patient Monitoring program (RPM).
    • Act as a direct liaison between the patient and the office/provider.
    • Scheduling and confirming appointments in office for the patient.
    • Have a full understanding of each patient's individual care plan and making sure they are following it.
    • Record patient data daily and alert healthcare providers of abnormal reports.

    Care Manager Qualifications:

    • At least 1 year of clinical work experience as a Medical Assistant
    • At least 1 year of EMR experience; eClinicalWorks preferred
    • High level of professionalism
    • Must live in Arizona
    • Excellent written and verbal communication skills
    • Strong customer service skills
    • Medical Assistant Course Completion Diploma or CMA designation required
