    In this role, you will conduct in-home assessments on enrolled Medicare Advantage members, providing annual clinical evaluations to identify and reduce health risks for patients and coordinating follow-up care to improve health and well-being.

    You will have the ability to achieve work-life balance with flexible scheduling offered where providers can flex their time over the 7-day work week, no on-call, no weekends, and no holidays required.

    Primary Responsibilities:
    • Conduct in-home assessments on health plan members, averaging 45-60 minutes in length, including past medical history, review of systems, medication review, evidence-based practice screenings, and vitals checks.
    • Check vitals, conduct physical exams, and perform point-of-care testing as needed.
    • Identify diagnoses and formulate lists of current and past medical conditions using clinical knowledge and judgment.
    • Communicate findings to inform member's primary care physician of potential gaps in care.
    • Educate members on disease processes, medication, and compliance.


    Required Qualifications:
    • National certification (ANCC or AANP) and active, unencumbered NP license in the state of assignment.
    • Family, geriatrics, adult, adult-gerontology primary, or adult-gerontology acute care certification.
    • Access to reliable transportation to travel to client and/or patient sites within a designated area.


    Preferred Qualifications:
    • 1+ years of clinical experience in their highest level of education, clinical setting.
    • Experience in gerontology, cardiology, internal medicine, or endocrinology.
    • Home Health care or home visit experience.


    Knowledge, Skills, and Abilities:
    • Excellent administrative, organizational, and verbal skills.
    • Professional image and behavior.
    • Solid communication skills with the geriatric or Medicare population.
    • Ability to work independently and maintain accurate records.
    • Detail-oriented and dependable.
