- Conduct in-home assessments on health plan members, averaging 45-60 minutes in length, including past medical history, review of systems, medication review, evidence-based practice screenings, and vitals checks.
- Check vitals, conduct physical exams, and perform point-of-care testing as needed.
- Identify diagnoses and formulate lists of current and past medical conditions using clinical knowledge and judgment.
- Communicate findings to inform member's primary care physician of potential gaps in care.
- Educate members on disease processes, medication, and compliance.
- National certification (ANCC or AANP) and active, unencumbered NP license in the state of assignment.
- Family, geriatrics, adult, adult-gerontology primary, or adult-gerontology acute care certification.
- Access to reliable transportation to travel to client and/or patient sites within a designated area.
- 1+ years of clinical experience in their highest level of education, clinical setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home Health care or home visit experience.
- Excellent administrative, organizational, and verbal skills.
- Professional image and behavior.
- Solid communication skills with the geriatric or Medicare population.
- Ability to work independently and maintain accurate records.
- Detail-oriented and dependable.
