    Job Description

    The Case Management Team Lead plays a crucial role in ensuring the quality of case management services delivered to our patient population. This individual will work under the direction of the Manager and Director of Case Management, providing direct case management services as needed and overseeing the services provided by other case managers and social workers.

    Responsibilities:

    • Assume accountability for the quality of case management services delivered to patients
    • Provide direct case management services as needed
    • Oversee services provided by other case managers and social workers
    • Assist with the coordination and orientation of new staff members
    • Contribute to the evaluation of staff performance

    Requirements:

    • Active Registered Nurse (RN) licensure in Indiana or a compact licensed state
    • Three years of relevant clinical experience

    Preferred Qualifications:

    • Case Management Certification
    • BSN or educational plan in place
    • Recent clinical experience in specific areas required for specialty clinical groups or clusters
    • Leadership experience

    Estimated Salary: $90,000 - $120,000 per year, depending on experience and location.
