    HumanEdge Health is seeking a dedicated

    Travel Pharmacist

    for a 13-week assignment at a reputable facility in Florence, SC.

    This role supports inpatient pharmacy operations, with an emphasis on clinical knowledge, medication reconciliation, and patient safety in accordance with current healthcare regulations and standards.


    Position Summary:
    Under the supervision of the Director, the Pharmacist will oversee and participate in all aspects of hospital pharmacy services. This includes preparing and dispensing medications, maintaining regulatory compliance, performing clinical consultations, and contributing to interdisciplinary care planning.


    Key Responsibilities:
    Ensure accurate dispensing, labeling, and documentation of all pharmaceuticals

    Manage sterile product preparation, parenteral solutions, and emergency medications

    Perform monthly inspections of drug storage and patient care areas

    Maintain pharmacy inventory, order medications, and manage controlled substances in accordance with state and federal laws

    Conduct medication reconciliation for admissions and discharges, including communication with patients and families

    Provide discharge counseling and education on prescribed medications

    Monitor patient drug therapy for contraindications, allergies, drug interactions, and appropriate dosing

    Support antimicrobial stewardship, culture review, and pharmacokinetic/anticoagulation dosing management

    Participate in P&T Committee meetings and medication use reviews

    Maintain
    EPIC system documentation and billing records

    Adhere to and enforce all safety standards and infection control protocols


    Required Qualifications:
    Active

    Pharmacist license

    in South Carolina (or compact state if applicable)

    Minimum

    2 years of recent hospital pharmacy experience


    EPIC
    system experience required

    Clinical knowledge in antimicrobial stewardship and therapeutic drug monitoring

    Strong communication skills for patient/family interactions and interdisciplinary collaboration

    Proficiency in medication reconciliation and discharge planning

    Familiarity with JCAHO standards and quality improvement initiatives
