- Review medication orders and patient profiles for accuracy, appropriateness, and potential interactions.
- Provide drug information and counsel patients on medication use, side effects, and precautions.
- Collaborate with healthcare providers to optimize medication therapy and patient outes.
- Monitor medication therapy and patient responses, adjusting as necessary to achieve desired outes.
- Participate in medication reconciliation processes to ensure continuity of care.
- Maintain accurate records of medications dispensed, interventions made, and patient consultations.
- Oversee the storage, handling, and disposal of medications in pliance with regulatory standards.
- Participate in quality improvement initiatives and medication safety programs.
- Stay informed about developments in pharmacy practice, regulations, and drug therapies.
- Utilize pharmacy technology systems effectively, including Cerner and Omnicell.
- Provide support and guidance to pharmacy technicians and other staff members as needed.
- Active pharmacist license in California.
- Doctor of Pharmacy (Pharm.
- Prior experience in a hospital or clinical pharmacy setting preferred.
- Strong knowledge of pharmacy laws, regulations, and standards of practice.
- Excellent munication, interpersonal, and teamwork skills.
