- Provide support and training to small groups of individuals served in accordance with program guidelines
- Assist participants with personal care, mobility, positive behavior interventions, and medication management
- Help individuals set and achieve personal goals, encouraging participation in various activities to promote independence and inclusion
- Implement individual support plans, document progress, and performance regularly
- Plan and facilitate skill development classes, recreational activities, community volunteer initiatives, and ensure participant safety at all times
- Offer crisis intervention support when needed, promptly reporting incidents to the Program Manager
- Ensure compliance with Human Rights, Medicaid, CARF, and other regulatory requirements
- Assist participants with transportation to and from community events
- Provide feedback on participant goals, behavior approaches, personal enrichment, and social integration
- Participate in meetings to support program development and implementation
- Complete assigned documentation accurately and in a timely manner
- Perform other duties as assigned
