- Lead and evaluate all case management operations, including model design, staffing, onboarding, competencies, and performance
- Manage FTE allocation, skill mix, and productivity to meet departmental goals
- Ensure proper execution of care coordination and discharge planning workflows, with consistent documentation and timeliness
- Oversee core case management functions including utilization review, transitions of care, and interdisciplinary collaboration
- Partner with the CFO and Division CM Leader to achieve financial and operational targets
- Use data analysis, KPIs, and process improvement strategies to drive sustained performance improvements
- Ensure full compliance with policies, procedures, and regulatory requirements
- Build strong relationships with CMO, CNO, ancillary leaders, compliance, and legal teams
- Provide leadership, coaching, and accountability for both direct and indirect reports
- Uphold organizational mission, values, and code of conduct
- 3+ years of recent acute hospital case management experience (within the last year)
- 2+ years of case management leadership
- Bachelor's degree required
- Master's degree (Nursing, Health Administration, Business Administration) preferred
- RN license or LCSW required
- Hospital discharge planning experience required
