- Intake/Admissions:
- Coordinates and facilitates the admissions process for patients transferring from other facilities, including methadone dose verification and prescription monitoring reporting.
- Schedules appointments, maintains medical records, and serves as back-up to patient services representative and medical assistant
- May be assigned to work with the clinic provider to complete admission and annual physicals. May be required to obtain and record patient vitals, obtain laboratory specimens (including phlebotomy) and complete other assigned tasks within scope of practice as directed.
- Assists medical providers with patient office visits, including putting patients in rooms, conducting necessary screenings, recording vital signs including pulse rate, blood pressure, height and weight, respiratory rate, temperature, and enters information in patient's electronic medical record
- Collection of selected data about the health status of the patient.
- Performs blood draws and sends samples to lab for processing
- Performs select clinical tests and demonstrates proper specimen collection techniques
- Performs EKGs using proper technique and procedure
- Dispensing medication: administers and dispenses medication as prescribed by the prescriber and in compliance with applicable regulations.
- Assessments/Specimen collection: completes direct observation of client prior to dosing including general physical presentation and mental status to confirms the patient is appropriate to receive the daily oral dose. Collects urinalysis specimens and other samples, views lab results, and informs supervisor of any discrepancies.
- Counseling: assumes counseling responsibilities in health-related areas, including but not limited to prenatal care, child and nutritional care and general hygiene.
- Record keeping and documentation: documents clearly and completely services provided, the patient's response and any change in condition noted. All documentation must be completed in accordance with Root Center policy, applicable CARF accreditation standards, Federal and State regulations.

Position Summary
Under the supervision of the Nursing Supervisor, the Admissions Registered Nurse (RN) has responsibility for the coordination of all medical services and assists with the day-to-day operations of the Admissions department, consistent with policies and procedures of the Root Center for Advanced Recovery, CARF OTP, Primary Care standards and CT-DPH regulations.
The role is responsible for assisting patients and performing or assisting with all clinical services available at the clinic, including administering and dispensing medication and patient assessments.
The Admissions RN will provide clinical, technical, and administrative support to the Admissions medical team.Essential Functions
Responsibilities include, but are not limited to:
Current active license in the State of Connecticut as a Registered Nurse with at least one year of nursing experience and demonstrated ability and sensitivity to work with patients within the scope of this rehabilitation program.
