Job summary

The position of RN Care Manager at Summa Health Medical Group involves using advanced nursing processes to identify high-risk patients and improve health outcomes through population health-based platforms.

Responsibilities include assessing, planning, educating, monitoring evaluating coordinating services for patient populations and their families.

Qualifications

  • Bachelor of Science in Nursing (BSN)
