    PCC Essential Job Duties:

    Scheduling and performing Medicare annual wellness visits for assigned providers.

    Assisting and scheduling patients to maintain up to date preventative services.

    Work with a multi-disciplinary team to manage care of chronic medical conditions.

    Education, Training & Experience

    Must have active IN LPN license. ( Must have graduated from an accredited nursing program)

    Must have active BLS certification
