Job Summary:

We are seeking a skilled Registered Nurse to join our team of healthcare professionals in the Heart Failure unit. This individual will be responsible for assessing patients' physical and psychosocial conditions on an ongoing basis throughout their care continuum. They will develop personalized plans of care reflecting individualized patient problems and interventions to achieve desired outcomes.

• Evaluates the effectiveness of each plan of care
• Revises plans as needed
• Makes referrals where indicated
