    Purpose of Your Job Position


    The primary purpose of your job position is to coordinate nutritional care of residents by completing nutritional observations/reviews, developing and implementing care plans, and documenting dietary information about residents under the supervision of the Registered /Licensed Dietitian.

    Refers residents to the Registered/Licensed Dietitian as appropriate, in accordance with current federal, state, and local standards, guidelines and regulations, and Solaris HealthCareâ€s established policies and procedures.

    • Nutritional Assessment Responsibilities
    • Completes nutritional observations/reviews, MDS, CAA's and care plans under the supervision of the Registered Dietitian.
    • Completes progress notes.
    • Visits residents routinely to determine effectiveness of interventions and to update food preferences and evaluate food intake.
    • Checks residents progress and determines effectiveness of nutritional care plans.
    • Promotes and assists in implementation of liberalized diets.
    • Uses sound professional judgement and standards of professional practice at all times.
    • Under the supervision of the Dietitian, reviews and completes the required documentation on residents nutritionally at risk.
    • Follows house supplement procedure recommending enhanced foods before supplements. Evaluates the effectiveness of supplements on a routine basis.
    • Reviews weight records routinely and communicates variances to interdisciplinary team.
    • Under the supervision of the Dietitian, makes enteral feeding and dialysis recommendations that are consistent with procedures.
    • Ensures timely follow-up on physician recommendations.
    • Communicates resident nutrition and hydration relevant changes in condition during the 24-hour Clinical Report/Stand-Up Meeting.
    • Uses current standards of dietetic practice in the implementation of nutritional care.
    • Participates in interdisciplinary team meetings, serves as Dining Room Monitor as required.

    Education

    Registration with the Academy of Nutrition and Dietetics.

    Experience

    At least one year experience in long term care, preferred.
